We have been informed by our Paying Agent that payments made for personal injury or wrongful death from a Qualified Settlement Fund are exempt from the W-8 requirement.

Please note that although the W-8 form is not required, the Plan Documents still require the Settlement Facility to obtain certain documents prior to releasing payments such as proof of foreign residency and Estate documentation for deceased claimants. We will continue to send written requests for this information as claimants become approved for payment.
