If you have submitted your Proof of Manufacturer form and supporting documents and have received a Notice of Status Letter identifying deficiencies with your claim, you may complete and send a Request for Review of Additional Proof of Manufacturer Information along with additional records to support your claim and return them to the Settlement Facility for review.
Only new records will be considered for a Review of Additional Information (RAI) review. Duplicate records or records that have already been reviewed will not be reviewed again.
